The landmark DiRECT trial 5-year follow-up (published 2025) showed 13% of participants maintained T2D remission (HbA1c < 6.5% off medication) at 5 years following a structured weight management program. Those maintaining >10 kg weight loss had 64% remission rates, establishing that T2D remission can be sustained long-term without pharmacotherapy in select patients.
Caloric restriction (800 kcal/day total diet replacement for 12-20 weeks) followed by structured food reintroduction and long-term weight maintenance. Remission driven by reduction of ectopic fat in the liver and pancreas, restoring beta cell function and hepatic insulin sensitivity.
